SIR Phred is a Safety Investigation process guiding users through:

• Basic, uncomplicated occurences
• Standard, more complex occurences
• Major Significant Occurences with Safety Issues Reviews

Basic Investigations can be started by anyone. It is a streamlined 4 step process. Here's a Basic flow chart.

Major incidents are investigated in 2 parts; the Significant Occurence and the Safety Issues Review. Click for a Significant Occurence flow chart.

Significant Occurence
Details soley what happened and any Immediate Actions that were taken to mitigate further risks from the Significant Occurence. This report is meant to be distributed outside the company.

Safety Issues Review
Reviews each factor that allowed the Significant Occurence to happen. Recommendations are created along with an action plan to ensure it will not happen again. This report is intended for internal use only.
