Significant Occurrence
Details solely what happened and any Immediate Actions that were taken to mitigate further risks from the Significant Occurrence. This report is meant to be distributed outside the company.

Safety Issues Review
Reviews each factor that allowed the Significant Occurrence to happen. Recommendations are created along with an action plan to ensure it will not happen again. This report is intended for internal use only.
