4 Questions Create & Implement the Solution
What is the overall plan?
Describe the overall plan for implementing permanent corrective and preventive actions.
Who needs to be notified of the permanent solution?
List the people and functions that are impacted by the plan.
How did you verify that this will prevent reoccurrence?
Explain what you did to prove that implementing the plan will prevent the problem from happening again. Describe how the solution was tested to make sure it works.
How will you verify that the problem will not happen again?
Explain when and how you will confirm that the problem does not happen again in the future. What monitoring system will notify you if it does happen again?

The database will tell you what happened with previous solutions to similar problems. This will help you avoid the same pitfalls while designing better solutions.

Organizations have multiple, often conflicting priorities. Whether it is a small solution, requiring just an OK, or a large capital investment, presenting all the facts in a clear manner helps people make the right decision.
Hold report outs to present the entire problem and solution. This is a valuable learning exercise and increases the quality of problem solutions across the organization.

Create, Track and Report on Actions
At each stage as the 8D has progressed, actions have been created and tracked. This feature of the PHRED system is most important in this implementation phase.
Actions can be viewed from either the problem report page or the Action Summary Report.
