At the heart of each PHRED system is a robust and easy to use root cause analysis tool. Like all PHRED systems, we use questions that guide users through a rigorous reasoning process. It progresses in stages from:

1. Grasping the situation. The more time spent here, the better your fishbone and ( 5 why) root cause chains

2. Do you need to put in a temporary fix to stem the bleeding?

3. Building your why chains

a. What are the possible causes
b. Create why chains ( the 5 +/- Whys)
c. Verify with data either a dead end or the actual root cause

5. Preventing reoccurrence and making sure the countermeasure is shared
