8D Steps

By individual problem, users determine if the issue should be closed at Issue Identification or if it is worth investigating to root cause and permanent solution through the 8D process.

Issue Identification information is carried forward to the 8D investigation. Problems are investigated through a rigorous, question-based reasoning process. Data to support reasoning is attached. Problem knowledge is captured in the database as the 8D progresses.

The system guides users through the 8D steps:
  • Using a Team Approach (D1)
  • Describe the Problem (D2)
  • Temporary Fix (D3)
  • 5 Why Root Cause Analysis (D4)
  • Creating & Implementing the Solution (D5 & D6)
  • Preventing Reoccurrence, Closing the Loop & Auditing (D7 & D8)
